发病超24小时缺血性脑卒中患者机械取栓疗效研究OACSTPCD

Objective To explore safe and effective of thrombectomy for ischemic stroke caused by occlusion of anterior circulation large vessels beyond 24 hours after onset.Methods Patients with anterior circulation thrombectomy were retrospective-ly included.According to onset time,they were divided into early time window group(≤6 h),late time window group(6～24 h)and beyond 24 hours group(＞24 h).The baseline,prognosis and surgical complications of three groups were compared.And fur-ther comparative analysis was made by propensity score and matched pair of outcome indicators.Results Total 680 patients were included,of which 12 were beyond 24 hours.There was no significant difference in the proportion of good prognosis and the propor-tion of death between patients with mechanical thrombectomy and those within the time window(P＞0.05).After propensity matc-hing analysis,there was no difference in good prognosis and death rate between patients with over time window and those with early time window(P＞0.05).Compared with patients in the late time window group,there was no difference in good prognosis and death rate between the patients in the late time window group(P＞0.05).Conclusion Thrombectomy may be safe and effective in patients with anterior circulation acute ischemic stroke whose onset time is more than 24 hours.This conclusion needs to be fur-ther verified by high-quality clinical studies.
