非肌少症老年人群血清酰基肉碱、溶血磷脂酰胆碱与握力的相关性研究OA北大核心CSTPCD
The correlation of acylcarnitine and Lys phosphatidylcholine with grip strength and gait speed in non-sarcopenia elderly population
目的 本文初步探讨非肌少症老年人群中血清代谢物(血清酰基肉碱、溶血磷脂酰胆碱)与握力等重要运动功能指标的相关性.方法 采用连续入组抽样法选取2021年6-12月在北京医院参加体检的182例非肌少症老年人群为研究对象,基于靶向液相色谱-串联质谱测量血清中12种酰基肉碱、溶血磷脂酰胆碱和甘氨酸等代谢物,对比握力下降组和握力正常组之间上述血清代谢物的水平差异,研究上述血清代谢物与握力、步速、骨骼肌质量指数(skeletal muscle mass in-dex,SMI)等运动评估指标的相关性,并通过Logistic受试者操作特征曲线(receiver operating characteristic curve,ROC曲线)分析评估关键血清代谢物对老年人握力和步速下降的评价效果.结果 非肌少症受试者的平均握力和步速分别为(31.91±5.72)kg和(1.01±0.31)m/s.与握力正常组相比,握力下降组的平均握力下降10.10 kg,步速下降0.13 m/s(t=-2.363,P=0.019),短链脂酰肉碱(简称C3)平均升高0.020 μmol/L(t=2.222,P=0.028)、溶血磷脂酰胆碱(LPC18∶0)平均升高 3.443 μmol/L(t=2.136,P=0.034)、甘氨酸升高 0.33 μmol/L(t=2.489,P=0.017).Pearson相关性分析显示,C3、十四烷酰左旋肉碱(简称C14)水平与运动功能呈较明显负相关性.进一步尝试发现4种代谢物(C3、C14、LPC18∶0、甘氨酸)联合年龄和体质量指数(body mass index,BMI)组合,在非肌少症老年人群中握力下降的评价效果,优于单纯使用年龄和BMI的组合.曲线下面积(area under the curve,AUC)分别为0.754(95%CI:0.658~0.849,P<0.001)、0.650(95%CI:0.545~0.754,P=0.011).结论 非肌少症老年人群中握力、步速下降与血清C3、C14;LPC18:0和甘氨酸升高存在正相关性,有望进一步在队列研究中探讨其应用于老年握力下降的预测评估价值.
Objective Early identification and support of high-risk groups in clinically non-sarcopenia healthy elderly population has potential screening value.The correlation of serum acylcarnitine,Lys phosphatidylcholine metabolites with grip strength and gait speed in non-sarcopenic elderly population was studied.Methods A total of 182 non-sarcopenia elderly people who participated in physical examination in Beijing Hospital from June to December 2021 were selected as the research subjects by convenience sampling method.12 metabolites of acylcarnitine,Lys phosphatidylcholine,amino acids and other metabolites in serum were measured based on targeted liquid chromatography/tandem mass spectrometry,and the differences of metabolites concentration between the reduced grip strength group and the normal group were compared.In the subsequent analysis,step speed was added as a secondary indicator to study the correlation difference between metabolites and exercise assessment indicators such as grip strength,walk speed and(skeletal muscle mass index)SMI,and the ability of key metabolites to distinguish grip strength and walk speed decline in the elderly was evaluated by Logistics(receiver operating characteristic curve,ROC curve).Results The average grip strength of the subjects decreased by 20.33%(31.91±5.72)kg;The average walking speed was(1.01±0.31)m/s,decreased by 34.07%;The concentrations of C3,C14,LPC18:0 and glycine in decreased grip strength group were significantly higher than those in normal group(P<0.05);Compared with the normal grip strength group,the grip strength of the subjects in the lower grip strength group decreased by 10.10 kg,and the pace decreased by 0.13 m/s(t=-2.363,P=0.019).C3 was increased by 0.020 μmol/L(t=2.222,P=0.028),LPC18∶0 was increased by 3.443 μmol/L(t=2.136,P=0.034),and glycine was increased by 0.33 μmol/L(t=2.489,P=0.017).No significant difference was found in the rest.Pearson correlation analysis showed that C3 and C14 showed opposite trends in different populations and the correlation was higher than C3,C14,LPC:18∶0,glycine combined with age and body mass index(BMI),the effect of grip strength decline in non-muscular elderly population was better than that of age and BMI alone.The area under curve(AUC)was 0.754(95%CI:0.658-0.849,P<0.001)and 0.650(95%CI:0.545-0.754,P=0.011),respectively.The four metabolites combined with age and BMI were more effective.Conclusions Decline of grip strength and serum C3,C14 in non-sarcopenia aged population;LPC18∶0,glycine has a positive correlation with grip strength decline in non-sarcopenia elderly population,and it is expected to further explore its value in predicting grip strength decline in elderly people in cohort studies.
