损伤性髋关节炎粘连状态可引发局部剧烈疼痛,导致受损关节活动受限,进而出现废用性肌萎缩、肌力下降等表现,严重影响患者生活质量,早期治疗能够有效控制病情发展.推拿能够直接作用于病变部位,安全且无副作用,因此探索关节粘连状态的推拿手法对于解除关节粘连状态、恢复关节功能活动具有重要意义.于天源教授在将中医学经络理论与西医学相结合的基础上,根据多年临床经验总结出"强则松之,塞则通之,瘀则去之,凝则动之,收则散之"的治疗原则,提出以"松筋、通经,祛瘀,助动,宣散"为核心的关节粘连治疗手法.本文通过介绍于天源教授治疗损伤性髋关节炎的手法操作与内涵,推动构建关节粘连状态的推拿特色治疗方案,为学科发展奠定基础.

The adhesion state of traumatic hip arthritis can cause severe local pain,resulting in limited movement of the damaged joint,and then appear disuse muscular atrophy,muscle strength decline and other manifestations,which seriously affect the quality of life of patients.Early treatment can effectively control the progression of the disease.Tuina can directly act on the affected area,safe and without side effects.Therefore,it is of great significance to explore the tuina technique of joint adhesion to relieve joint adhesion state and restore joint functional activities.Professor Yu Tianyuan has been engaged in acupuncture-moxibustion and tuina for more than 30 years.On the basis of combining the meridian theory of traditional Chinese medicine with modern medicine,and based on years of clinical experience,he summarized the treatment principle as"Relax the area if it is rigid;Unblock the area if it is blocked;Resolve the area if it is in stasis;Move the area if it is congealing;Spread the area is it is in contracture".He proposed"sinew relaxing,meridian unblocking,stasis eliminating,movement assisting,and diffusion"for the treatment.This article introduces Professor Yu Tianyuan's manipulation and connotation in treating traumatic hip arthritis,promoting the construction of a unique tuina treatment plan for joint adhesion,and laying the foundation for the development of the discipline.
