目的 探讨配偶照顾负担在脑卒中偏瘫患者及其配偶二元应对间的中介作用,为制订减轻配偶照顾负担的干预措施提供参考.方法 2021年6月至2022年6月,采用方便抽样方法选取华北理工大学附属医院和开滦总医院收治的311例脑卒中偏瘫住院患者及其配偶作为研究对象.采用一般资料调查表、二元应对量表(DCI)及照顾者负担量表(ZBI)对其进行调查.结果 脑卒中偏瘫患者及其配偶二元应对得分分别为(100.05±19.02)分和(86.03±16.62)分,患者配偶照顾负担得分为(59.49±13.90)分.患者配偶照顾负担得分与患者及其配偶二元应对得分均呈负相关(r1=-0.719,P<0.001;r2=-0.790,P<0.001),患者二元应对得分与患者配偶二元应对得分呈正相关(r=0.840,P<0.001).模型拟合结果显示,患者配偶照顾负担在患者及其配偶二元应对间起中介作用(β=0.264,P<0.001),中介效应占总效应的30.14%.结论 脑卒中偏瘫患者二元应对水平偏低,其二元应对水平可直接影响或通过其配偶照顾负担间接影响其配偶的二元应水平.医护人员可通过制订针对性干预措施以提高脑卒中偏瘫患者及其家属的二元应对水平,并降低其配偶的照顾负担,促进其心理健康.

Objective To explore the mediating effect of spouse care burden on the relationship between post-stroke hemiplegia patients'dyadic coping and their spouses'dyadic coping,aiming to provide a reference for formulating interventions to reduce the care burden of spouses.Methods From June 2021 to June 2022,311 inpatients with post-stroke hemiplegia,along with their spouses,were selected by convenience sampling.These patients were admitted to the North China University of Science and Technology Affiliated Hospital and Kailuan General Hospital.They were investigated using the General Information Questionnaire,Dyadic Coping Inventory(DCI)and Zarit Burden Interview(ZBI).Results The dyadic coping scores of patients and their spouses were(100.05±19.02)points and(86.03±16.62)points,respectively;the care burden score of spouses was(59.49±13.90)points.The care burden scores of spouses were negatively correlated with the dyadic coping scores of patients and their spouses(r1=-0.719,P<0.001;r2=-0.790,P<0.001),and the dyadic coping scores of patients were positively correlated with those of spouses(r=0.840,P<0.001).Model fitting results showed that the care burden of spouses exerted a mediating effect on the relationship between patients'dual dyadic coping and their spouses'dyadic coping(β=0.264,P<0.001),accounting for 30.14%of the total effect.Conclusion Patients with post-stroke hemiplegia have a low level of dyadic coping,which directly affects the dyadic coping level of their spouses or indirectly through the care burden of their spouses.Healthcare providers can improve the dyadic coping levels of patients with post-stroke hemiplegia and their spouses,reduce the care burden of their spouses,and promote their mental health by developing targeted interventions.
