目的:探讨Smart ERA技术评估子宫内膜容受性在不孕症的应用效果.方法:选取赣州市妇幼保健院于 2021 年 1 月—2023 年 12 月体外受精-胚胎移植(in vito fertilization-embryo transfer,IVF-ET)的 76 例不孕症患者的临床资料,根据妊娠结果分为妊娠组 46 例和未妊娠组 30 例.超声诊断仪系统内配备Smart ERA技术检测内膜厚度、内膜形态、内膜运动方向、内膜下血流分型、子宫螺旋动脉血流、子宫内膜容积、血管指数(vascularization index,VI)、血流指数(flow index,FI)、血管血流指数(vascularization flow index,VFI)、螺旋动脉阻力指数(resistance index,RI)、螺旋动脉搏动指数(pulsatility index,PI)、收缩期峰值血流速度/舒张末期血流速度(systole/diastole,S/D),采用ROC曲线预测不孕症患者妊娠结局.结果:妊娠组螺旋动脉RI、螺旋动脉PI、S/D均显著低于非妊娠组(P<0.05),而子宫内膜容积、VI、FI和VFI均显著高于非妊娠组患者(P<0.05),两组子宫内膜厚度、子宫内膜形态、内膜运动方向、内膜下血流分型比较差异均无统计学意义(P>0.05).螺旋动脉RI、螺旋动脉PI、S/D、VI、FI、VFI及子宫内膜容积预测妊娠结局的AUC分别为 0.818、0.789、0.897、0.904、0.896、0.983、0.780.结论:超声检测参数可以有效评估子宫内膜的容受性,并预测不孕症患者的妊娠结局.这些发现支持Smart ERA技术在评估和治疗不孕症中的应用价值.

Objective:To explore the effectiveness of Smart ERA technology in evaluating endometrial receptivity for the application in infertility.Method:Clinical data from 76 infertility patients who underwent in vitro fertilization-embryo transfer(IVF-ET)at Ganzhou Women and Children's Health Care Hospital from January 2021 to December 2023 were selected.Based on pregnancy outcomes,patients were divided into a pregnant group(46 cases)and a non-pregnant group(30 cases).The ultrasound diagnostic system was equipped with Smart ERA technology to detect endometrial thickness,endometrial morphology,direction of endometrial movement,sub-endometrial blood flow classification,blood flow of the uterine spiral arteries,endometrial volume,vascularization index(VI),flow index(FI),vascularization flow index(VFI),resistance index(RI)of the spiral arteries,pulsatility index(PI)of the spiral arteries,and the systole/diastole ratio(S/D),utilizing ROC curves to predict pregnancy outcomes in infertility patients.Result:The RI of the spiral arteries,PI of the spiral arteries,and S/D values were significantly lower in the pregnant group than those in the non-pregnant group(P<0.05),while endometrial volume,VI,FI and VFI were significantly higher in the pregnant group than those in the non-pregnant group(P<0.05).No significant differences were observed between the two groups in terms of endometrial thickness,morphology,direction of movement,and sub-endometrial blood flow classification(P>0.05).The areas under the curve for predicting pregnancy outcomes based on RI of the spiral arteries,PI of the spiral arteries,S/D,VI,FI,VFI and endometrial volume were 0.818,0.789,0.897,0.904,0.896,0.983 and 0.780 respectively.Conclusion:Ultrasound parameters effectively assess endometrial receptivity and predict pregnancy outcomes in infertility patients.These findings support the application value of Smart ERA technology in the assessment and treatment of infertility.
