经颅磁刺激在脑卒中后吞咽障碍康复中应用的研究进展OA

脑卒中是常见的脑血管疾病,脑卒中后患者会遗留运动、感觉、吞咽等功能障碍.其中,吞咽障碍易使患者发生吸入性肺炎、营养不良以及电解质紊乱等并发症,严重影响患者生活和疾病的转归.经颅磁刺激作为一种无创的神经调节技术,已被多项研究证实可显著改善脑卒中后吞咽障碍.本文归纳总结了脑卒中后吞咽障碍的发病机制、经颅磁刺激治疗研究现状及其可能机制,发现临床上采用经颅磁刺激治疗时使用的频率、强度、部位、模式等参数的不同,其产生的治疗效果也有所不同.根据双相平衡恢复理论,理想的经颅磁刺激方案应取决于患者功能结构保留度、中风类型以及分期等.此外发现,小脑刺激、经迷走神经刺激等刺激方式也对脑卒中后患者的吞咽功能的改善有效果,但今后需进一步研究.

Stroke is the most common cerebrovascular disease,and patients may experience various functional impairments such as movement,sensation,and swallowing.Post-stroke dysphagia can easily lead to complications such as aspiration pneumonia,malnu-trition,and electrolyte disorders,seriously affecting the patient's life and disease outcome.Transcranial magnetic stimulation,as a non-invasive neuromodulation technique,has been proven by multiple studies to significantly improve swallowing disorders after stroke.This article summarizes the pathogenesis of swallowing disorders after stroke,the current research status and possible mecha-nisms of transcranial magnetic stimulation therapy.It is found that the therapeutic effects of transcranial magnetic stimulation therapy vary depending on the frequency,intensity,location,mode and other parameters used in clinical practice.According to the bimodal balance-recovery model,the ideal transcranial magnetic stimulation regimen should depend on the patient's functional and structural preservation,stroke type,and stroke staging.In addition,novel stimulation methods such as cerebellar stimulation and vagus nerve stimulation also have an effect on improving swallowing function in stroke patients,and further research is needed.
