目的:探究中性粒细胞细胞外陷阱网(neutrophil extracellular traps,NETs)调控的破骨细胞生成对根尖周炎炎症及骨吸收的作用.方法:C57BL/6 小鼠通过髓腔直接暴露法建立根尖周炎模型,然后将小鼠随机分为模型组和干预组,对干预组小鼠皮下注射DNase I(10 mg/kg).健康小鼠作为空白组.检测模型组和空白组小鼠根尖周区域NETs水平.比较小鼠牙龈指数、根尖周病变、病变区域中性粒细胞和破骨细胞数量、釉牙骨质界(CEJ)到牙槽嵴顶(ABC)距离及根尖周围组织肿瘤坏死因子(tumor necrosis factor,TNF)-α、白细胞介素(interleukin,IL)-6、IL-1β水平.检测病变根尖周围组织骨保护素(osteoprotegerin,OPG)、核因子κB受体活化因子配体(receptor activator of nuclear factor-κB ligand,RANKL)、核因子κB受体活化因子(receptor activator of nuclear factor-κB,RANK)蛋白水平.结果:模型组牙龈指数、根尖周病变面积、根尖周病变区域中性粒细胞数量和NETs含量及根尖周围组织TNF-α、IL-6、IL-1β mRNA水平显著高于空白组,而干预组上述指标显著低于模型组(P<0.05).相比于空白组,模型组根尖周病变区域破骨细胞细胞数量及周围组织RANKL、RANK蛋白水平显著增加,OPG蛋白水平显著减少,而干预组破骨细胞数量及RANKL、RANK蛋白水平显著少于模型组,OPG蛋白水平显著高于模型组(P<0.05).结论:根尖周炎组织中NETs水平显著升高,其可能通过调控破骨细胞分化促进根尖周病变及骨吸收.

Objective:To explore the effect of osteoclast genesis regulated by neutrophil extracellular traps(NETs)on inflammation and bone resorption in periapical periodontitis.Methods:The periapical periodontitis model of C57BL/6 mice was established by direct exposure of the medullary cavity,and then the mice were randomly divided into model group and in-tervention group.The mice in the intervention group were subcutaneously injected with DNase I(10 mg/kg).Healthy mice were used as blank group.The levels of NETs in the periapical areas of the model group and the blank group were detected.The gingival index,periapical lesions,the number of neutrophils and osteoclasts in the lesion area,the distance from the ce-mento-enamel junction(CEJ)to the alveolar bone crest(ABC),and the levels of tumor necrosis factor(TNF)-α,interleukin(IL)-6 and IL-1β in the periapical tissues were compared.The protein levels of osteoprotegerin(OPG),receptor activator of nuclear factor-κB ligand(RANKL)and receptor activator of nuclear factor-κB(RANK)in the tissues around the lesion apex were detected.Results:The gingival index,periapical lesion area,the neutrophil counts/the content of NETs in the periapical lesion area and the levels of TNF-α,IL-6 and IL-1β mRNA in the periapical tissues of the model group were significantly higher than those of the blank group,while the above indexes of the intervention group were significantly lower than those of the model group(P<0.05).Compared with the blank group,the number of osteoclasts,the levels of RANKL and RANK pro-tein in surrounding tissues in the periapical lesion area of the model group were significantly increased,and the level of OPG protein was significantly decreased,while the number of osteoclasts and the levels of RANKL and RANK protein in the inter-vention group were significantly lower than those in the model group,and the level of OPG protein was significantly higher than that in the model group(P<0.05).Conclusion:The level of NETs in periapical tissues is significantly increased,which may promote periapical lesions and bone resorption by regulating osteoclast differentiation.
