Interpretation of the Society of Thoracic Surgeons Clinical Practice Guidelines on the Management of Neonates and Infants with Coarctation(2024)and comparison with domestic expert consensus
本文旨在解读美国胸外科医师学会(Society of Thoracic Surgeons,STS)发布的2024版新生儿和婴儿孤立性主动脉缩窄(coarctation of the aorta,CoA)的外科手术管理指南,并探讨其与国内专家共识的差异.指南通过系统回顾文献,提出了一系列针对新生儿和婴儿孤立性CoA的手术管理推荐,包括首选治疗方法、手术时机、手术方法和入路、修复技术和策略,以及神经保护方式等.与国内共识相比,STS指南在手术指征、手术方法、技术和策略、治疗时机以及语言表述上存在明显差异.STS指南更侧重于手术方法的标准化和个体化治疗,国内共识则强调急诊手术指征和尽早手术的重要性.本文的发现为国内临床医师提供了宝贵参考,有助于推动新生儿和婴儿孤立性CoA诊疗规范的完善和发展.
This paper aims to interpret the 2024 edition of the Society of Thoracic Surgeons(STS)Clinical Practice Guidelines on the Management of Neonates and Infants with Coarctation,and to discuss the differences between these guidelines and the domestic expert consensus.The guidelines provide a series of rec-ommendations for the surgical management of isolated coarctation of the aorta(CoA)in neonates and infants based on a systematic review of the literature.These recommendations include preferred treatment methods,tim-ing of surgery,surgical techniques and approaches,repair techniques and strategies,and methods of neuroprotec-tion.Compared to the domestic consensus,the STS guidelines show significant differences in surgical indica-tions,methods,techniques and strategies,timing of treatment,and terminology.The STS guidelines place more emphasis on the standardization of surgical methods and individualized treatment,while the domestic consensus highlights the importance of emergency surgical indications and early surgery.The findings of this paper provide valuable reference for domestic clinicians and contribute to the improvement and development of diagnostic and therapeutic standards for isolated CoA in neonates and infants.
