糖尿病肾脏疾病(DKD)是一类以进行性肾组织纤维化为特征的肾脏疾病.在明确糖尿病为肾损害病因并排除其他原因引起的慢性肾脏病(CKD)前提下,1型和2型糖尿病患者出现持续性白蛋白尿,或估算肾小球滤过率进行性下降大于 3个月,或肾活检符合DKD病理诊断,可诊断为DKD.细胞通讯网络(CCN)家族蛋白是一组细胞间基质蛋白,参与调解生长、凋亡、衰老、自噬、迁移、分化和炎症等重要细胞病理生理改变.新近研究显示,CCN家族蛋白在DKD大鼠肾脏模型中表达显著上调,提示其在DKD肾小管上皮细胞发生上皮-间充质转化(EMT)和肾间质纤维化进展中发挥重要作用.本文综述了CCN家族蛋白介导DKD肾脏损伤的作用及机制,以探索DKD潜在的治疗靶点.

Diabetes kidney disease(DKD)is a kind of kidney disease characterized by progressive renal tissue fibrosis.On the premise that diabetes is the cause of kidney damage and chronic kidney disease(CKD)caused by other reasons is excluded,type 1 and type 2 diabetes patients can be diagnosed with DKD if they have persistent albuminuria,or if the estimated glomerular filtra-tion rate has declined for more than 3 months,or if renal biopsy conforms to the pathological diagnosis of DKD.The cell communi-cation network(CCN)family of proteins is a group of intercellular matrix proteins involved in regulating important cellular patho-physiological changes such as growth,apoptosis,aging,autophagy,migration,differentiation,and inflammation.Recent studies have shown that the CCN family proteins are significantly upregulated in the DKD rat kidney model,suggesting that they play an important role in the epithelial-mesenchymal transition(EMT)of DKD renal tubular epithelial cells and the progression of renal in-terstitial fibrosis.This article reviews the role and mechanism of CCN family proteins in mediating renal injury in DKD,in order to explore potential therapeutic targets for DKD.
