我国成年人膳食炎症指数与2型糖尿病发生风险的队列研究
A Cohort Study Investigating The Association Between Dietary Inflammatory Index and The Risk of Type 2 Diabetes Among Chinese Adults

Abstract
[Objective]To investigate the association between the energy-adjusted dietary inflammatory index(E-DII)and the risk of type 2 diabetes in adults across 15 provinces(autonomous regions/municipalities)in China.[Method]A total of 8 687 adults,who were free of diabetes at baseline,were selected from the"China Health and Nutrition Survey"conducted between 2009 and 2018.The E-DII was calculated using dietary data collected via a three-day consecutive 24-hour recall method,supplemented by household weighing and record-keeping,and analyzed using the nutrient residual model.Demographic,economic,lifestyle,and health status information was gathered through structured questionnaires and physical examinations.Participants were divided into five quartile groups(Q1-Q5)based on their E-DII scores for each corresponding year.The association between E-DII and the risk of type 2 diabetes was assessed using a robust estimation stratified Cox proportional hazards regression model.A restricted cubic spline(RCS)model was employed to further investigate the dose-response relationship between E-DII levels and the risk of type 2 diabetes mellitus.[Result]The study included 8 687 participants with a mean baseline age of 50.77 years(SD=13.08).Participants were followed for 3 to 9 years,accumulating a total of 48 837 person-years(mean follow-up duration of 5.62 years per participant).During the follow-up period,777 cases of type 2 diabetes were identified.The observed E-DII scores ranged from-4.13 to+5.17,with a mean score of 1.34.Among the participants,3 883 were males(44.7%)and 4 804 were females(55.3%),with baseline E-DII scores of 1.33(SD=1.05)and 1.30(SD=0.99),respectively.After adjusting for relevant confounding factors using a robust estimation stratified Cox proportional hazards regression model,males in the Q4 group exhibited a marginally significant higher risk of developing type 2 diabetes compared to those in the E-DII Q1 group(HR=1.295,95%CI:0.962,1.742).Females in the Q5 group had a significantly increased risk of type 2 diabetes by 38.1%(HR=1.381,95%CI:1.016,1.876).Analysis of the restricted cubic spline model indicated that,after adjusting for relevant confounding factors,there was no significant nonlinear relationship between E-DII levels and the risk of type 2 diabetes in either males or females(P for nonlinearity>0.05).[Conclu-sion]There is a positive association between E-DII and the risk of type 2 diabetes among adults in 15 provinces of China,however,no clear dose-response relationship was identified.关键词
