营养风险指数与老年糖尿病足溃疡患者治疗后复发的关系
Relationship Between Nutritional Risk Index and Relapse in Elderly Diabetic Foot Ulcers After Treatment

Abstract
[Objective]To investigate the relationship between nutritional risk index(NRI)and postoperative recurrence of diabetic foot ulcers in elderly patients.[Method]A total of 121 DFU patients who underwent VSD in the Department of Hand and Foot Surgery in Jiaozhou Central Hospital of Qingdao from February 2021 to February 2024 were included in the DFU group,52 patients with T2DM were included in the T2DM group,and 52 healthy volunteers were included in the control group.The DFU group was divided into relapse group(47 cases)and non-recurrence group(74 cases)according to the 1-year follow-up.Multivariate Logistic regression analysis was conducted to analyze the factors affecting patients'recurrence,and ROC curve was used to analyze the predictive value of NRI for patients'recurrence after treatment.[Result]There were significant differences in NRI among the three groups(P<0.05),the lowest NRI was in DFU group,followed by T2DM group,and the highest NRI was in control group.The proportions of Wagner grades 3 to 4,DPN,ankle-brachial index difference,HbA1c,CRP and WBC in the relapsed group were higher than those in the non-relapsed group(P<0.05),and the Alb and NRI in the relapsed group were lower than those in the non-relapsed group(P<0.05).Multivariate Logistic regression analysis showed Wagner grade[OR=5.571,95%CI:2.505-12.394],ankle-brachial index[OR=4.222,95%CI:1.791-9.954],HbA1c[OR=2.151,95%CI:1.460-3.171],Alb[OR=0.651,95%CI:0.549-0.772],CRP[OR=1.302,95%CI:1.159-1.462],WBC[OR=1.542,95%CI:1.240-1.917]and NRI[OR=0.341,95%CI:0.197-0.591]were relapse factors after treatment(P<0.001).ROC curve analysis showed that NRI had high efficacy in predicting relapse after treatment,with AUC of 0.809(95%CI:0.733～0.886),sensitivity of 0.649,specificity of 0.957,Yodon index of 0.606,and optimal cut-off value of 82.57(P<0.001).[Conclusion]NRI is an independent predictor of recurrence in elderly diabetic foot ulcers after treatment.Patients with low NRI have a higher risk of recurrence.Clinical monitoring of NRI can be used to assess the recurrence risk of patients and take appropriate intervention measures.关键词
